Referral to Central Clinic for Psychotherapy (need both of the below filled out/signed):
Referral to Surgeon (use ".transrefsurgeon" to have them contact surgeon for consult/financing).
Forms needed:
PCP Letter to Surgeon (will need to edit this and print on letterhead)
Letter to try to get electrolysis covered by insurance (patient will need to call insurance
company to get fax number that this can be sent to)
If needed, more robust letter to help justify coverage of surgery here
