Use to send secure chat requests to front desk staff to either reschedule or convert to phone visit. 

Asks for date and time of original appt, patient name, and specify phone visit or reschedule. There is one additional spot for items you would like staff to relay to patient. They request that you keep this very brief (they are not supposed to be triaging anything). For example, if you are requesting a BP f/u to reschedule, you may have staff tell them to let you know if they need any refills on BP medications.
