The FMC Transitional Care Clinic is designed to ensure timely follow up for patients discharged from the hospital, as an opportunity to review medications and prevent re-hospitalization. TCC typically involves longer patient care visits and often utilizes Medicare’s “TCM” (Transitional Care Management) billing codes.
