Objectives:

1)    Explain health gaps between cancer treatment/survivorship and role of primary care oncology practices in addressing these gaps.  

2)    Explore particular care needs for survivors of breast, colorectal, prostate and head/neck cancers based on ACS survivorship guidelines.

3)     List the recommended monitoring for post-cancer care for survivors. 

4)    Apply primary care oncology concepts in patient care experiences, EMR tools, resource list, and/or peer teaching.

The Residents will be assigned to Participating Site for 12-18 half day sessions either over the period of 2 weeks, or twice monthly over a 6-month period.
