Experience a Direct Primary Care or Value Based Care model as a 6 or 12 month longitudinal elective

Experience patient care, physician schedule/responsibilities, and office work flow in a practice using an alternative healthcare delivery model, such as direct primary care, value based care or an accountable care organization.
Compare and contrast the practice’s model of healthcare delivery with a fee-for-service traditional model with regards to payment stucture.
Examine the practice’s methods for collecting and analyzing patient data and the methods used to improve outcomes and/or ensure quality care.
Describe the impact the healthcare delivery model may have on physician well-being (consider: physician schedule and after hours coverage, financial (dis)incentives, EHR and paper workload, frequency and types of patient interactions, and any other factors you know to negatively or positively impact your own well-being).
Illustrate the ways the practice engages (or disengages) with the surrounding community. This may include education events, addressing health-related social needs, advocacy, partnerships, or something else.
